Top-Pirex drugs are indicated for topical treatment in the infection outside the eyeball and the appendages of the eye due to sensitive bacteria: eyelid inflammation, conjunctivitis, tear bag inflammation, keratitis.
Pharmacokology
Tobramycin is an antibiotic belonging to the aminoglycoside group that has a bactericidal effect on the mechanism of inhibiting the synthesis of protein in sensitive bacteria by not being opposite to ribosom subunits.
The effect of Tobramycin is aerobic, staphylococcus aerobic bacteria, including strains that produce penicillinase and methicillin resistance, and some aerobic gram -positive bacteria. In Vietnam, Tobramycin works well with Salmonella Typhi, Shigella Flexneri, Proteus spp. Some bacteria have increased resistance to Tobramycin including: E.coli, Entobacter, Pseudomonas Aeruginosa. There is a cross -resistance between Tobramycin and Gentamycin but there are about 10% of the Gentamycin strains that are also sensitive to Tobramycin.
Tobramycin is often used in the form of sulfate. The drug is not absorbed by oral, so it is often intramuscularly or intravenously. In addition, Tobramycin is also used in the form of water or 0.3% eye ointment for eye infections.
Less -oriented drugs are associated with plasma proteins, diffuse mainly into extracellular fluids and diffuse easily into the inner ear fluid. Half of the lifetime of serum in normal people is 2-3 hours, and up to 93% of the dose is excreted into urine within 24 hours in the form of unchanged.
Dynamic pharmacokinetics
When the eyes are top-pirex, the system exposure is very low. It is impossible to measure the concentration of Tobramycin in the plasma of 9 out of 12 small people per drop x 4 times/day for 2 consecutive days. The highest concentration measures 0.25 mcg/ml, 8 times smaller than the concentration of 2 mcg/ml, the known concentration is below the threshold of toxicity to the kidney.

Dosage

For mild and medium infections: Put one drop into the eye, every 4 hours.
In severe infections: Put one drop into the eyes, once an hour until improved, then reduce the dose gradually before stopping the drug.
When using Top - Pirex, should monitor the response of bacteria for local antibiotic therapy appropriately.

Side Effects
When using top-pirex drugs you may experience unwanted effects (ADR).

Contraindicated
Top-Pirex drugs contraindicated in the following cases:

Be cautious when used for people with renal diseases before, vestibular disorders, disability in the cochlea, after favorable surgery and other conditions that reduce neurotransmitter.
As with other antibiotics, prolonged use can produce resistance bacteria, including fungi. If there is superinfection, appropriate therapy should be applied.
The ability to drive and operate machinery
is similar to other eye drops, temporarily blurred vision and vision disorders that can affect the ability to drive and operate machinery. In the case of blurred vision, it is necessary to wait until it is clearly visible to continue driving and operating machinery.
Pregnancy
Do not take medicine for pregnant women.
The period of breastfeeding
The drug can be used for breastfeeding women.
Drug interaction
If the Tobamycin eye drops are simultaneously used with other aminoglycosides antibiotics used in the body, then the likelihood will increase toxicity to the body. Therefore, the drug concentration is needed. Avoid not to keep the serum concentration over 12 mcg/ml.
Although small tobramycin eye absorption into the body with very low content is still needed to monitor the lung condition in patients taking anesthetic.
Concomitance use tobramycin with nerve -muscle blockers will be nervous - muscle and respiratory paralysis.

Storage
Store in dry places, avoid light, temperature not exceeding 30ºC.
Do not use after opening a bottle for 1 month. Close the jar after each use.
