Pharmacy
Acetaminophen is an effective analgesic - antipyretic drug that can replace Aspirin, but acetaminophen does not have the effect of inflammatory treatment. With equal doses, acetaminophen has analgesic, reducing fever similar to Aspirin.
Acetaminophen reduces body temperature in fever but rarely reduces body temperature in normal people.
Acetaminophen, with treatment, less impact on the cardiovascular and respiratory system, does not change acid-base balance, does not cause irritation, scratches or stomach bleeding as when using aspirin and salicylate.
When overdose of acetaminophen, n-acetyl-p-benzoquinoneimin metabolism is toxic to the liver.
Normal dose, acetaminophen well tolerated does not have many serious side effects like aspirin. However, acute overdose (over 10 g) will damage the liver.
pharmacokinetics
absorption: Acetaminophen is quickly absorbed from the digestive tract with peak concentration in plasma reaching about 10-60 minutes after drinking.
Distribution: Acetaminophen is distributed mostly in the body's tissues. Acetaminophen through the placenta and present in breast milk. Integrated with negligible plasma proteins at conventional treatment concentrations but binding will increase as the concentration increases. Selling time is about 1-3 hours.
Metabolism and elimination: Acetaminophen mainly metabolizes in the liver and eliminates urine in the form of glucuronid conjugate (60-80%) and sulphate conjugate (20-30%). Less than 5% is excreted in constant form. A small part (less than 4%) is metabolized under the effect of Cytochrom P450 into metabolic. In case of poisoning due to high doses, the amount of metabolites increases and is detoxified by combined with glutathion.

What to do when overdose?
Symptoms
Nausea, vomiting, anorexia, pale, abdominal pain.
Overdose of acetaminophen can cause liver failure. Excessive doses of over 10 g in adults (lower dose in alcoholics) and over 150 mg/kg of children's weight, single dose may cause liver cell decomposition that leads to complete necrosis and non -recovery, metabolic acidosis, brain disease leading to coma or death.
Emergency treatment
Early diagnosis is important in the treatment of overdose acetaminophen. When severe poisoning, it is important to treat positive support. Gastric lavage is needed in any case, preferably within 4 hours after drinking. The main detoxification is sulfhydryl compounds, perhaps partly due to the addition of glutathion reserves in the liver.
n-acetylcysteine works when taken or intravenously. Must give the drug immediately if less than 36 hours after taking acetaminophen.
Treatment with n-acetylcystein is more effective when giving the drug for less than 10 hours after taking acetaminophen.
When drinking, dilute N-acetycystein solution with water or drinks without alcohol to achieve a 5% solution and must be taken within 1 hour after mixing.
Give N -acetylcystein oral at the first dose of 140 mg/ kg, then another dose, each dose of 70 mg/ kg each 4 hours apart. Termination of treatment if acetaminophen test in plasma shows the risk of low liver toxicity.
What to do when forgetting 1 dose?
Not recorded.
