Uses
Indications
vancomycin are indicated in severe infections, kidney and heart disease:
Vancomycin has a bactericidal effect by inhibiting biosynthesis processes, in the stage earlier than ß-lactam antibiotics. Vancomycin also affects cell membranes and RNA synthesis of bacteria.
Vancomycin works well in Gram (+) bacteria and anaerobic, including staphylococcus aureus, including Staphylococcus aureus, Staphylococcus epidermidis (including heterogeneous methicilin strains), streptococcus, including Streptococcus pneumoniae (including penicillin resistance) Agalactiae, Streptococcus Pyogenes, Streptococcus Bovis, colon bridge (e.g. Enterococcus Faecalis) and Clostridiae. Gram -negative bacteria are all resistant to vancomycin.
Dynamic pharmacokinetics
Intravenous drugs to treat systemic bacterial infections. - After intravenous injection, the drug is distributed in extracellular fluids.
Bacterial inhibition concentration is achieved in pleural fluid, pericardial fluid, ascites, discharge, urine, in the abdominal discharge, in the urinary tissue.
The distribution of the drug is about 60 liters/70 kg.
The half -life of the drug from 3 to 13 hours, prolonged in the kidney damage. When the plasma concentration is 10 - 100 µg/ml, about 55% of vancomycin dose is linked to plasma. The drug is almost no metabolism.
Vancomycin is excreted mainly through the kidneys, about 70-80% of the dose is eliminated in the form of unchanged urine within 24 hours in people with normal kidney function. Vancomycin cannot be removed by hemorrhage or abdominal separation method.

How to use
Dissolve the drug with 10 ml of the solvent that contains 50 mg/ml. Dilute the above solution into 100 ml of solvent that is slowly intravenously for 60 minutes. Vancomycin solution can be diluted with 0.9%sodium chloride solution, 5%dextrose, ringer lactate or ringer lactate and 5%dexerrose.
It is necessary to avoid rapid intravenous injection and during the transmission must be closely monitored to detect hypotension if occurred.
Dosage
Dosage is calculated according to vancomycin base.
For people with normal kidney function:
Adults: 1 vialycin bottle of 500 mg/time, every 6 hours. Or 2 vans vancomycin 500 mg/time, every 12 hours. Endotological inflammation due to staphylococcus: must be treated for at least 3 weeks.
To prevent endocarditis in patients with penicillin allergies, there is a high risk of tooth extraction or a surgical procedure: Give a single dose of 2 vanicomycin 500 mg vials in combination with Gentamicin, intravenous infusion before doing the procedure.
If the patient has a gastrointestinal or urinary - genital surgery: Give a single dose of 1 g of vancomycin combined with Gentamicin, intravenously before the procedure. Repeat after 8 hours.
Children: 10 mg/kg body weight/time, every 6 hours.
Infants: The first dose of 15 mg/kg, followed by 10 mg/kg. Every 12 hours of the first week of age.
Every 8 hours in the following weeks to 1 month of age.
Prevention of endocarditis in children is at high risk of penicillin allergies that need tooth extraction or other surgical procedures: 20 mg/kg starts 1 hour before the procedure and repeats 8 hours later.
Gastric - intestinal or genital tract surgery: 20 mg/kg starts 1 hour before surgery, and is accompanied by a 2 mg/kg of intramuscular or intravenous gentamicin, starting for half an hour to 1 hour before surgery. Inject these 2 drugs after 8 hours.
People with impaired renal function and elderly:
Dosage according to the following table:
100
For patients this type the first dose of 15 mg/kg, to maintain concentration, it is necessary to maintain the maintenance dose 1.9 mg/kg/24 hours. Then every 7 - 10 days use 1 dose 1 g.
Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist. What to do when overdose?
Treatment when using overdose drugs: supporting treatment, maintaining glomerular filtration level. Removing vancomycin by the less effective separation method.
dialysis through membrane and adsorbent helps increase vancomycin excretion speed.
In an emergency, call the 115 emergency center immediately or go to the nearest local health station.
What to do when you forget 1 dose? However, if the time to relax with the next dose is too short, skip the dose and continue the calendar of the drug. Do not use double dose to compensate for missed dose.

For patients with impaired renal function, the dose adjustment and closely monitoring the kidney function.
Avoid simultaneous use with high -toxic drugs on the kidneys and hearing. Concomitant use with aminoglycosides, causing high risk of toxicity to the kidneys, but it is still necessary to combine drugs in cases of serious infections that threaten life.
It is necessary to measure the hearing function many times to prevent toxic risk to hearing when using vancomycin. - Vancomycin causes tissue irritation, compulsory intravenous injection. Pain, pain and necrosis occur if intramuscularly or injected.
