Pharmacokinus
Furosemid is a diuretics that conducts sulfonamid diuretic in a strong, fast, dosage -dependent effect group. The drug acts in the branches of the Henle straps, so it is classified as diuretics.
The main mechanism of action of Furosemid is to inhibit the Na+/K+/2Cl-2-clinical transport system in the thick segment of the branch on the Henle straps, increasing the excretion of these electrolytes accompanied by increased water export exercises.
The drug also reduces Na+, Cl– reabsorption and increases the excretion of K+ in the distance and can work directly on the nearby tube. Furosemid increases the elimination of Ca2+, Mg2+, hydrogen, ammonium, bicarbonate and maybe even the kidney phosphate. Losing a lot of potassium, hydrogen and chlorine can cause metabolism.
Due to reducing plasma volume, it can cause hypotension but usually only mitigate.
Furosemid has the effect of renal vasodilation, reducing the impact of the kidney and blood flow through the kidney after taking the drug. When taking high doses in patients with chronic renal impairment, the filtration rate of the glomeruli may increase temporarily. If the urinary tract is excessive due to the drug that reduces plasma volume, the blood flow may occur and reduces the rate of glomerular filtration.
Dynamic pharmacokinetics
Furosemid quickly absorbed through the gastrointestinal tract, bioavailability about 60 - 70% but absorbed and erratic, affected by the form of medicine, disease processes and the presence of food. However, whether you take medicine when you are hungry or full, the diuretic benefits are similar. In patients with heart failure, Furosemid absorption is even more erratic. Born can be reduced to 10% in kidney disease, increasing slightly in liver disease. When taken, the effect appears quickly after 1/2 hours, reaches the maximum effect after 1-2 hours and maintains the effect from 6 - 8 hours.
up to 99% of blood furosemids attached to plasma albumin. The free (non -mounted) Furosemid part is higher in people with heart disease, kidney failure and cirrhosis. Furosemid is excreted mainly through urine, largely in the form of non -metabolic. Selling time from 30 minutes to 120 minutes in normal people, prolonged sale time in infants and patients with liver and kidney failure. Furosemid passed the placenta and distributed into breast milk. The clearance of Furosemid does not increase when hemolysis.
Before taking Vinzix 40mg vinphaco medicine for edema and hypertension (5 blisters x 50 tablets)
How to use
oral medication.
Dosage
Adults and children over 12 years old
Treatment of edema:
The normal initial dose is 40 mg daily in the morning. The diuretic effect can last about 4 hours. Therefore, the time to use the drug can be adjusted according to the patient condition. The maintenance dose is 20 mg daily or 40 mg daily. In severe cases, the dose can be increased to 80 mg daily.
Hypertension treatment:
The usual dose is 20 - 40 mg, twice daily. If there is no clinical response, the dose may increase to 40 mg, twice daily. When taking 40mg, twice a day without blood pressure, adding an anti -hypertension drug is different from increasing the dose of Furosemid. Strictly monitor blood pressure when using Furosemid alone or in combination with other anti -hypertension drugs, especially when starting treatment.
Hypercalcemia treatment:
Adults increased slightly with blood calcium concentration: Take 120 mg/day. In severe cases, switch to injection.
Children under 12 years old
Other forms of preparation should be used for this object.
Elderly
The elimination of drugs takes place more slowly. Therefore, the dose adjustment is needed to meet the treatment requirements.
Patients with impaired liver function, reduced blood protein
Need to adjust the dose when taking the drug in these patients.
High -dose therapy
Acute or chronic renal failure:
high doses of furosemids are used to treat other therapy for acute or chronic renal failure, including abdominal or artificial kidney. In some patients, Furosemid may delay the demand for appraisal, increase the distance to be fertilized, shorten the time of hospitalization or allow the water to put water into the body a little more free.
Adults:
may start with oral 80 mg/day and an increase of 80 - 120 mg/day until satisfactory. When it is necessary to have immediate diuretic benefits, the oral dose starts 320 - 400 mg/day. Some patients have taken 4 days. While using high -dose Furosemid therapy, it is necessary to check the water - electrolyte balance. Especially in shocked people, blood pressure and blood volume must be measured to adjust before starting this therapy. This high -dose therapy is contraindicated in renal failure due to toxic drugs for kidney or liver and in renal failure in combination with liver coma.
Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist. What to do when overdose?
Symptoms
Dehydration, reduced blood volume, hypotension, electrolyte imbalance, hypokalemia, alkali infection.
Handling
In return, the amount of water and electrolytes has been lost. Check regularly in serum, carbon dioxid and blood pressure. Must ensure adequate drainage in patients with congestion of urine from the bladder (such as prostate hypertrophy). Hematoparoology does not increase Furosemid excretion.
What to do when you forget 1 dose? However, if the time to relax with the next dose is too short, skip the dose and continue the calendar of the drug. Do not use double dose to compensate for missed dose.
Side Effects
When using Vinzix 40 mg Vinphaco 5x50, you may experience unwanted effects (ADR).
Very common, common, ADR> 1/100
Uncommon, 1/1000 Kidney and urinary: small urine, non -autonomous urine, urinary tract congestion. Very rare, rare, ADR Nervous system: mental disorders, parcels, confusion, headache. Vascular disorders: vasculitis, thrombosis, shock. Other: Paracos, transaminase increases. Not determined frequency, ADR: Instructions on how to handle ADR When experiencing side effects of the drug, it is necessary to stop using and notify the doctor or go to the nearest medical facility for timely treatment.
Warnings
Before using the drug you need to read the instructions carefully and refer to the information below.
Contraindicated
Vinzix 40 mg Vinphaco 5x50 contraindications in the following cases:
Money coma/coma is related to cirrhosis or brain disease.
Digitalis poisoning.
Be cautious when using
lower blood pressure and/or reduce blood volume, acid - base balance disorders: These symptoms should be treated before using Furosemid. Hypotension with symptoms such as dizziness, fainting or loss of consciousness may occur in patients being treated with Furosemid, especially with the elderly.
Need to adjust the dose for patients with impaired liver function, lower blood protein.
Be careful when taking drugs in patients with impaired liver, kidney, liver and kidney syndrome, diabetes, elderly people, difficult urination patients, at risk of obstruction of the urinary tract including prostate hypertrophy, gout, patients with risk of hypotension.
The ability to drive and operate machinery
The drug can cause unwanted effects such as headache, hypotension, dizziness, nausea, vomiting. Therefore, do not drive, operate machinery or work on high when experiencing unwanted effects of the drug.
Pregnancy
The ability to cause teratogenic and embryo effects of Furosemid in humans has not been well known. There is very little evidence of safety when using high doses of furosemid during pregnancy, although animals show no harmful effects. Do not use for pregnant women unless the benefits for patients are greater than the risk that may occur with the fetus.
breastfeeding period
using Furosemid during breastfeeding is at risk of excretion or into breast milk. Should stop breastfeeding if taking the drug is necessary.
Interactive drug
vasodilators and antihypertensive drugs: Use with Furosemid will increase the risk of hypotension. Furosemid should be stopped or reduced the dose before starting treatment with these drugs.
Anti -psychotic drugs: Furosemid hypotension increases the risk of toxicity on the heart.
Anti -arrhythmias include Amiodaron, Disopyramid, Flecanaid, and Sotalol increases the risk of heart toxicity. The effects of lidocaine, tocainid or mexiletin may be opposed by Furosemid.
The drug involves extending the QT interval: The toxicity on the heart may increase due to hypotension or hypoglycemia caused by Furosemid.
cardiac glycoside: increases the risk of toxicity on the heart
