Uses
Indications
Virclath 500mg drug is indicated in the following cases:
Clarithromycin inhibits the synthesis of protein in sensitive bacteria by attaching the subunit of 50S ribosom, thus preventing the movement of aminocyl transfer-rna and inhibiting polypeptide synthesis. The acting position of Clarithromycin is also the effect of erythromycin, clindamycin, lincycin and chloramphenicol.
antibacterial spectrum
In vitro, Clarithromycin have similar or stronger effect than erythromycin for bacteria sensitive to erythromycin and also have anti -micro -microbiological activity (such as the non -typical Mycobacteria, toxoplasma).
sensitive bacteria
Gram -positive aerobic bacteria: Clarithromycin has a stronger printing effect than erythromycin for sensitive bacteria streptococci and staphylococci.
Clarithromycin also works on a few Gram -positive aerobic bacilli such as Listeria Monocytogenes and some Corynebacterium.
Gram -negative aerobic bacteria: In vitro, Clarithromycin are active for some Gram -negative bacteria such as Neisseria Gonorrhoeae and Moraxella (Branhamella) Catatrhalis, Haemophilus Influenzae, H. Parainfluenzae, Pasteurella Multocida. Clarithromycin has a stronger printing effect than erythromycin with legionella spp., Campylobacter spp. Bordetella pertussis. Clarithromycin works with most Helicobacter pylori strains, the drug works on Helicobacter pylori stronger than other macrolids.
Clarithromycin works stronger than erythromycin and azithromycin for mycobacteria including complex mycobacterium avium and M. Leprae.
Other aerobic bacteria: Clarithromycin works with Mycoplasma Pneumonia, Ureaplasma Urealyticum, Clamydia Trachomatis and some strains of C. Pneumoniae.
Anaerobic bacteria: In vitro, Clarithromycin are active with most peptococcus strains, peptostreptoccus, clostridium perfringens, propionibacterium acnes, Prevotella spp strains., Bacteroides Fragilis.
Other bacteria are sensitive to drugs including Toxoplasma Gondii, Gardnerella Vaginalis, Borrelia Burgdorferi and Cryptosporidis.
Metabolic substance 14 - Hydroxy Clarithromycin is active and can be in vitro in vital with mother medicine to significantly increase the activity of clarithromycin clinically for Haemophilus Influenzae, Legionella spp.
The situation of bacteria resistant to macrolid in Vietnam tends to increase rapidly. Bacteria resistant to erythromycin are often resistant to other macrolides including clarithromycin.
Staphylococcus and Streptococcus level resistance levels are 44% (ASTS, 1997) and of S.Pneumoniae are about 25% (ASTS.1996).
pharmacokinetic
absorption
Clarithromycin, when taken, is absorbed quickly through the gastrointestinal tract and is subject to the first metabolism at a high level that reduces the biological use of the mother medicine to about 55%. The absorption level is almost not affected by food. The peak concentration of clarithromycin and the main metabolites 14 - hydroxy clarithromycin is about 0.6 - 0.7microgam/ml, after taking a single dose 250mg.
In the dynamic equilibrium at the same dose level for the peak concentration of about 1microgam/ml. The pharmacokinetics of clarithromycin are not linear and dependent on doses. Large doses can create peak concentrations without proportion due to saturated drug metabolism.
Distribution
Clarithromycin and main metabolites are widely distributed and the concentration in tissue exceeds the serum concentration due to a part of the drug is collected into the cell.
Metabolism - Elimination
Metabolic drugs are high in the liver and release stool through biliary tract. A significant part is discharged through urine. About 20 and 30% in the order of the dose of 250mg and 500mg are discharged in the urine in the form of no metabolized. 14 - Hydroxy Clarithromycin as well as other metabolites are also discharged through urine.
The half -life of Clarithromycin is about 3-4 hours when the patient takes 250mg Clarithromycin, 2 times/day, and about 5-7 hours when the patient takes 500mg, 2 times/day. Half life is prolonged in patients with kidney failure. Before taking Virclath 500mg quimica treatment of allergies, pneumonia (4 blisters x 7 tablets)
How to use
oral drugs. Drink with cool boiled water, can be taken before or after meals.
Dosage
Clarithromycin is sustainable in acidic environment and absorbs well when there is or without food. Clarithromycin treatment time depends on the type of infection and the severity of the disease and usually lasts 7-14 days.
Adults
Respiratory and skin infections
Use 1 tablet/time, 2 times/day.
For patients with severe kidney failure
The dose is reduced to half to 250mg, 1 time/day or 250mg, 2 times/day in severe infections.
With mycobacterium avium intracellular (Mai)
Use 1 tablet/time, 2 times/day. Reduce the dose to 50% if the clearance is less than 30ml/min.
Children
Common dose
Use 7.5mg/kg body weight, 2 times/day to up to 500mg, 2 times a day.
Community pneumonia
Use 15mg/kg body weight, every 12 hours.
Clarithromycin is also used in combination with proton pump inhibitors and other drugs at a dose of 1 tablet/time, 3 times/day to eradicate Helicobacter pylori infection.
Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist.
What to do when overdose? If taken overdose, patients are hospitalized, gastric lavage and other supportive treatments. Clarithromycin concentration in serum is not affected by dialysis or peritoneal fertilizer.
What to do when forgetting 1 dose? However, if the time to relax with the next dose is too short, skip the dose and continue the calendar of the drug. Do not use double dose to compensate for missed dose.
Side Effects
When using Virclath 500mg, you may experience unwanted effects (ADR).
Common, ADR> 1/100
Allergic reactions at different levels from urticaria to anaphylaxis and Stevens - Johnson syndrome.
Uncommon, 1/1000 Instructions on how to handle ADR When experiencing side effects of the drug, it is necessary to stop using and notify the doctor or go to the nearest medical facility for timely treatment.
Warnings
Before using the drug you need to read the instructions carefully and refer to the information below.
Contraindicated
Virclath 500mg drug is contraindicated in the following cases:
Precautions when using
impaired renal and liver function.
The ability to drive and operate machinery
The drug does not affect the ability to drive or operate machinery.
Pregnancy
During pregnancy, only Clarithromycin is used when necessary and carefully monitored.
Breastfeeding period Drug interaction
The most important interaction of macrolid antibiotics with other drugs is because macrolid has the ability to inhibit metabolism in the liver of other drugs. Cytocrom P450 inhibitors are evident after taking Clarithromycin.
With epilepsy medications, Clarithromycin inhibits the metabolism of carbamazepin and phenytoin increases their side effects. Clarithromycin inhibits the metabolism of cisaprid leads to the distance of Q - T for a long time, torsion, ventricular vibration. Clarithromycin inhibits metabolism in theophylin's liver and increases theophylin concentration in plasma, leading to the risk of poisoning.
The drug also reduces the absorption of zidovudin. Dynamic studies have shown that clarithromycin and other macrolid antibiotics affect the metabolism of terfienadin, resulting in an increase in the accumulation of this drug.

Store in closed packaging, below 30 ° C. Avoid light.
