Vitamin B6 drugs are indicated in the following cases:
Treatment of acute poisoning (coma, convulsions) due to the overdose of Isoniazid, Cycloserin.
Treatment of genetic iron anemia.
Treatment of metabolic disorders: Treatment of oxalic acid hypertrophy in primary urine, primary urine homocystin, primary urinary cystathionin or urine with xanthurenic acid.
Pharmacokic
Vitamin B6 is a B vitamin -soluble vitamin, which exists in 3 forms: pyridoxal, pyridoxin and pyridoxamin. When entering the body, the body will be transformed into a pyridoxal phosphate activity and partly into pyridoxamin phosphate. These two substances act as coenzymes in the metabolism of protein, glucid and lipid. Pyridoxin participates in the synthesis of Aminobutyric Gamma acid (GABA) in the central nervous system and participates in Hemoglobulin synthesis.
Daily needs for children is 0.3-2 mg, adults about 1.6-2 mg and pregnant or lactating mothers are 2.1-2.2 mg. It is rare for vitamin B6 deficiency in humans, but it may occur in the case of absorption disorders, congenital metabolic disorders or drug disorders caused by drugs. When nutrition is deficient, there is less difficult case of a B vitamins, so vitamin supplements in the form of a mixture are more effective than single use.
The best is still improving the diet. Meat, fish, eggs, milk, liver, kidneys, vegetables, fruits are a rich source of natural pyridoxine. Increased body demand and vitamin B6 supplements may be necessary in the following cases: alcoholism, burns, congestive heart failure, prolonged fever, stomach removal, dialysis, thyroid glands, bacterial infections, intestinal diseases (such as diarrhea, bowel inflammation), malabsorption related to liver -bile diseases. For patients treated with isoniazid or women who take birth control pills, vitamin B6 needs more daily than usual.Pyridoxine deficiency can lead to iron -cell anemia, peripheral neuritis, seborrheic dermatitis, dry lips.
In some very rare babies with pyridoxine dependence syndrome, it is necessary to use large amounts of pyridoxine in the first week after giving birth to prevent seizures, also detected genetic iron -cell anemia responding to pyridoxine.
Some metabolic disorders: xanthurenic acid - urinary, cystathionin - primary urinary, increased oxalate - primary urinary (genetic): can respond to high doses of pyridoxin.
pharmacokinetics
absorption:
Vitamin B6 is easily absorbed through the gastrointestinal tract, which may be reduced in people with malabsorption syndrome or after gastric cutting. Normal concentration of plasma pyridoxine: 30-80 nanogam/ ml.
Distribution:
After drinking, the drug is mostly reserved in the liver, less than in the muscle and brain. Storage of Vitamin B6 body is estimated at about 167 mg. The main types of vitamin B6 in the blood are pyridoxal and pyridoxal phosphate, high links to protein.
pyridoxal through the placenta and concentration in fetal plasma is 5 times the mother's plasma concentration. The concentration of vitamin B6 in breast milk is about 150-240 nanogam/ ml after the mother takes 2.5–5 ml of vitamin B6 daily. After the mother takes less than 2.5 mg of vitamin B6 daily, the level of vitamin B6 in breast milk on average 130 nanogam/ml.
Metabolism:
In red blood cells, pyridoxin converts into pyridoxal phosphate and pyridoxamin converted into pyridoxamin phosphate. In pyridoxin phosphoryl turned into pyridoxin phosphate and converted into pyridoxal and pyridoxamin to quickly be phosphorylation. Riboflavin is necessary to convert pyridoxin phosphate into pyridoxal phosphate.
Era:
Pyridoxin's biological sale time is about 15-20 days. In the liver, pyridoxal transforms into 4-pyridoxic acid excreted into the urine. In cirrhosis, the rate of degradation may increase. Pyridoxal can be removed by hemolysis.
Before taking Vitamin B6 250mg Domesco drug treatment (20 blisters x 10 tablets)
How to use
Take oral use.
Dosage
Although Pyridoxin was previously considered non -toxic, even with high doses, but now shows that long -term high doses (for example, 2 g/day) to treat some diseases that can cause nervous effects, so it is necessary to consider benefits and harm.
Treatment of poisoning causes convulsions and/ or coma:
Do Isoniazid: Take 1 dose of pyridoxin with the amount of isoniazid, often with anti -convulsions if necessary.
Due to cycloserin overdose: 1 tablet/ day.
Treatment of genetic iron -cell anemia:
Take 1-2 tablets/ day.
If after 1-2 months of treatment, the disease is not transferred, it is necessary to consider another treatment.
If there is a response, pyridoxin can be reduced to 30-50 mg/day (using other forms of preparation to get the appropriate content). It may be necessary to treat this vitamin for a lifetime to treat anemia in people with this disease.
Treatment of metabolic disorders:
Treatment of oxalic acid in primary urine, primary urinary homocystin, cystathionin primary urinary or urine with xanthurenic acid: 1-2 tablets/ day.
If responded, prolonged treatment for life. Some patients with hygiene and oxalatimum of type 1 can be treated at lower doses (physiological doses or less than 100 mg/ day - use other forms of preparation to have an appropriate content).
Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist. What to do when using overdose?
pyridoxin is often considered non -toxic, but when high doses (such as 2 g/ day or more) lasts more than 30 days, which can cause sensory neurological syndrome, loss of coordination. Neuropathy syndrome may be caused by anatomical lesions of the nodes on the spinal cord nerve. Manifestations in the sense of position and tremor of the limbs, losing the combination of sensory movements gradually. Touching, distinguishing hot and cold and less painful, no muscle weakness.
How to handle:
If an overdose is needed to take the patient to a medical facility to treat symptoms. Stop using pyridoxin. After stopping pyridoxin, neurological dysplasia gradually improved and long -term monitoring saw a good recovery, can stop the drug for up to 6 months for the sensory nervous system to return to normal.
What to do when you forget a dose?
If you forget to take a dose, drink as soon as you remember. If the time is close to the next medication, skip the forgotten dose and take the next dose at the usual time. Do not use double dose to compensate for the forgotten dose.
Side Effects
When using vitamin B6, you may experience unwanted effects (ADR).
pyridoxin is usually not toxic. Using pyridoxine lasts for 10 mg/ day is said to be safe but using pyridoxin for a long time at a dose of 200 mg or more daily can cause neurological disease.
Unwanted Reaction Summary Table:
Agencies
Unwanted reactions
Drinking, drowsiness
digestive nausea and vomiting
AST increases
Using the dose of 200 mg/ day and long (over 2 months) can cause severe peripheral nerve inflammation, progressing from unstable appearance and numbness to the clumsy and numbness of the hand. This condition can recover when stopping the drug, although there is still more or less sequelae.
Other
allergic reactions
When experiencing side effects of the drug, it is necessary to stop using and notify the doctor or go to the nearest medical facility for timely treatment.
Warnings
Before using the drug you need to read the instructions carefully and refer to the information below.
Contraindicated
Vitamin B6 contraindications in the following cases:
Hypersensitivity to pyridoxin or any ingredients of the drug.
Be cautious when used
After a long time of using pyridoxin at a dose of 200 mg/day, it can be seen that the manifestation of neurotoxins such as severe peripheral neuropathy and severe sensory neuropathy.
Taking a dose of 200 mg daily, lasting over 30 days can cause pyridoxin dependence syndrome and cessation syndrome.
using pyridoxin for a long time with a daily dose of over 10 mg has not been shown to be safe.
The ability to drive and operate machinery
When high doses and long doses (over 2 months) can cause severe peripheral nerve inflammation, progressing from unsteady and numbness of the feet to the numbness and clumsy hands. So when this symptom does not operate machinery, driving a train, working on high and other cases.
Pregnancy
pyridoxin through placenta and evidence of safety when used for pregnant women.
Additional daily dose on daily needs is not harmful to the fetus.
Breastfeeding period
pyridoxin into breast milk.
No effect when used according to daily needs. High -dose pyridoxin has been used (600 mg/ day, divided into 3 times) to turn off milk, although it is usually ineffective.
Drug interaction
pyridoxin reduces the effect of levodopa in the treatment of Parkinson's disease, which does not occur for preparations as a mixture of Levodopa-Carbodopa or Levodopa-BenseraDazid.
Usually 200 mg of doses may cause 40-50% of the concentration of phenytoin and phenobarbiton in the blood in some patients.
Some drugs may increase pyridoxin demand such as hydralazin, isoniazid, penicilamine and oral contraceptives. Pyridoxin can lighten depression in women who take birth control pills.
Storage
In a cool dry place, avoid light, at temperatures below 30ºC.
