Uses
Indication
Voltaren Emulgel is indicated for external treatment for pain, inflammation and swelling in the following cases:
Pharmacokology
Pharmacological Group
Diclofenac is a nonsteroidal anti -inflammatory drug (NSAID), with analgesic, effective anti -inflammatory and antipyretic properties. The main mechanism of Diclofenac is to inhibit prostaglandin synthesis by cyclo-oxygenase 2 (COX-2).
Voltaren Emulgel is an anti -inflammatory and analgesic preparation. In case of inflammation and pain derived from trauma or rheumatism, Voltaren Emulgel 1.16% reduces pain and reduces swelling.
Dynamic pharmacokinetics
absorption
Diclofenac is absorbed through the skin proportional to the size of the treatment area, and depending on the total dose in place and the level of hydration of the skin.
The absorption of about 6% of Diclofenac dose after applying 2.5 g Voltaren Emulgel is over 500 cm of the skin surface, calculated from the total amount of excretion through the kidneys compared to the volume achieved from the voltaren tablet. Diclofenac absorption increased by 3 times if icy for 10 hours.
distribution
99.7% Diclofenac is connected to serum protein, mainly albumin (99.4%).
After applying Diclofenac Diethylamine gel on the spot to the hand and knee joint, Diclofenac concentration is measured in plasma, epidemic tissue and solution. Diclofenac's highest concentration in plasma is about 100 times lower than after using the same amount of diclofenac oral form. Diclofenac stored in the skin and acts as the source of reserves where there is a continuous release of drugs into the tissue below. From skin and subcutaneous tissue, Diclofenac prioritizes distribution and continues to exist deep in inflamed tissues (such as joints) more in the blood. Diclofenac is found in tissue at a concentration of 20 times higher than in plasma.
transformation
Diclofenac's biological conversion is related to once and several times via glucuronide, and a part of intact molecules.
Elimination
diclofenac and its metabolites are excreted mainly in urine.
The total body clearance of diclofenac from plasma is 263 ± 56 ml/min. Selling time in plasma is 1-2 hours. 4 metabolites, including both active metabolites, also have short half-life in plasma is 1-3 hours. A metabolic substance is 3’-hydroxy-4’-methoxy-diclofenac that has a much longer half-life but almost no activity.
Special patient group
Patients with kidney failure and liver failure
Diclofenac accumulation and its metabolites are unpredictable in the case of patients with renal failure.
Diet and metabolism of diclofenac in patients with chronic hepatitis or cirrhosis are similar to patients without liver disease.
Before taking Voltaren Emulgel Novartis gel for pain relief from injury or rheumatism (20g)
How to use
only use on the skin.
Depending on the size of the pain area to be treated, apply 2 g to 4 g of voltaren emulgel (the size of the cherry fruit to the lymph nodes) is sufficient to treat an area of about 400 - 800 cm.
After applying the drug, it is necessary to wash your hands properly except for the places being treated.
Duration of treatment depends on indications and clinical response.
DosageAdults and children aged 14 and over
Patients should consult a doctor if the condition does not improve or worsen within 7 days at the beginning of treatment.
Only for adults and children aged 14 and older.
Elderly patients (over 65 years old)
The usual adult dose of Voltaren Emulgel can be used.Apply medicine to the area of pain 3 or 4 times/day and massage gently.
Do not use Voltaren Emulgel for longer than 14 days for soft tissue lesions or low tissue rheumatism, or 21 days to reduce arthritis pain, unless the doctor's recommendations.
Note: The above dose is for reference only. Specific dosage depends on the condition and level of progression of the disease. For a suitable dose, you need to consult a doctor or medical specialist. What to do when overdose?
Signs and symptoms
The absorption of diclofenac is low after local use, so it is difficult to overdose.
However, the side effects are similar to the side effects observed in the case of the overdose of the voltaren tablet if you have to take Diclofenac on the spot (diclofenac diethylamine 1.16% gel form: 100 g tube corresponding to 1 g of diclofenac sodium).
Overdose treatment
In case of accidentally swallowing drugs that cause significant systemic side effects, should use common treatment by conventional methods to treat nonsteroidal anti -inflammatory drug poisoning.
Moreover the handling should be clinically indicated or as recommended by the National toxic centers if any.
Due to the low body absorption of diclofenac after local use, it is difficult to overdose.
What to do when you forget a dose? However, if close to the next dose, skip the forgotten dose and apply the next dose at the time as planned. Do not apply double the prescribed dose.
Side Effects
When using Voltaren Emulgel , you may experience unwanted effects (ADR).
The unwanted effects are listed below according to the body's organization classification and the frequency of appearance.
To classify the level of unwanted effects, use the following conventions: Very popular (≥1/10), popular (≥1/100,
The unwanted reaction was determined during the post -marketing period that was voluntarily reported from an unknown size population, the frequency of these unwanted effects appeared but was small or very small.
For each frequency group, the undesirable reaction according to the degree of severity decreases.
Warnings
Before using the drug you need to read the instructions carefully and refer to the information below.
contraindicated
Voltaren Emulgel contraindicated in the following cases:
Caution when using
cannot rule out the possibility of systemic side effects due to the use of Voltaren Emulgel if the preparation is used on wide skin areas and for a long time.
Voltaren Emulgel contains propylene glycol, which can cause mild, local skin irritation in some patients. Should be cautious when using the oral nsAID drugs because of the unwanted effect rate, especially the whole body side effects can increase.
Do not simultaneously use Voltaren Emulgel with other products that contain diclofenac.
Only apply Voltaren Emulgel on intact skin surfaces, no disease and skin without tearing or open wounds.
Do not leave the drug in contact with the eyes and mucosa, and not swallow.
Stop treatment if the skin is progressive after taking the drug.
Can use Voltaren Emulgel on the spot with non -closed tape but should not be used for air bandages.
Some stomach bleeding capabilities in people with a significant history of this condition have been reported in separate cases.
The ability to drive and operate machinery
Using topical voltaren does not affect the ability to drive and operate machinery.
Pregnancy
fertility
There is no data on the use of diclofenac on the spot and its effects on human fertility.
Pregnant women
Not enough data on the use of diclofenac for pregnant women. Only use diclofenac in the first two stages of pregnancy if evaluating the expected benefits with the risk of fetal risk. Like NSAIDs drugs, contraindicated using Voltaren Emulgel during phase 3 of pregnancy due to uterus, kidney failure in the fetus with subsequent amniotic fluid and/or early closing artery.
Nursing period
It is unclear whether diclofenac used on the spot is excreted in breast milk or not. Diclofenac should only be used during breastfeeding if evaluating the expected benefits with the risk of babies.
In case of mandatory indications, do not apply to the chest, do not use the drug in higher doses and the time for the medication is longer than the recommended dose.
Drug interaction
Due to the low body absorption when applied on the spot, the probability of drug interaction is very low.
Storage
Do not store over 30 ° C, avoid heat.
